These forms are entirely private, for your own personal use.
They are designed to give you a framework for jotting down ideas, aims and ambitions in time periods that are long enough for you to achieve something. The start/end of each semester is a logical time to review social and academic ambitions.
Make a list of possible goals. Highlight those you want to improve by the end of the Semester. Write down what you will do to reach these targets. (Be specific: eg don't write, "I will do more reading", write something like "I will spend 3 hours a week in the library taking notes from the reading". Similarly, do not put "I will be more chatty in discussion", try saying something like "I will listen to others and present my views succinctly in discussion."
Don't try to do too much, little goals are more achievable.
